Provider-led, coordinated healthcare that begins with you, the patient.

Your EKPCN team consists of Physicians, Nurse Practitioners, Registered Nurses, Allied Health Professionals, Clinic Staff, Midwives, and Aboriginal Care Navigators who work together to ensure all Elk Valley residents have a home base for their everyday healthcare needs.

PCN Allied Health and Nursing Services

Social Worker

PCN Social Workers can help individuals manage a variety of emotional and social concerns that impact their health and wellbeing. They can assist with housing and food security, mental health concerns, identity needs, and crisis and safety needs. They support connection of individuals with appropriate community services, assist with access to benefit services, and coordinate with individuals’ healthcare teams.

Mental Wellness Clinician

PCN Mental Wellness Clinicians provide proactive support, education, consultation, and referrals for those experiencing mild to moderate mental health and substance use conditions, including support for people with depression and anxiety.

Physiotherapist

PCN Physiotherapists provide support, education, and rehab pathways for individuals whose function and movement may be impacted by aging, disease, injury, environmental factors, and other health conditions.

Registered Dietitian

PCN Registered Dietitians provide individuals with comprehensive nutritional support. They can help individuals to:

  • Set personal goals for dietary changes;
  • Access practical nutrition advice related to chronic disease;
  • Manage digestive issues, celiac disease and food allergies;
  • Receive support during and after pregnancy;
  • Learn about healthy eating and nutrition for all ages.

Occupational Therapist

PCN Occupational Therapists offer supports that enable individuals to better do the things they need (and want) to do in both their work and everyday life. Occupational Therapists also can work with individuals to create a plan to self-manage stress, promote health, prevent illness, and manage chronic disease.

Registered Nurse

PCN Registered Nurses (RNs) provide holistic, person-centred, and culturally focused care throughout an individual’s healthcare journey. In collaboration with the healthcare team, the RN initiates, implements, and monitors a person’s healthcare plans and supports their goals.

Clinical Pharmacist

The Primary Care Clinical Pharmacist (PCCP) works directly with Family Physicians and Nurse Practitioners in the on-going care of patients with complex conditions to:

  • Prevent and resolve medication-related problems;
  • Educate patients about their medications;
  • Help remove barriers to patients in following prescription advice.

Aboriginal Care Navigator

PCN Aboriginal Care Navigator (ACN) work to support the delivery of culturally safe care. Aboriginal Care Navigator can be accessed by request through your family practice clinic, aboriginal health and wellness centre, or primary care provider. 

Services include:

  • Advocacy support: Facilitates patient, family, and community voice in patient care planning. Supports patient self-advocacy. Accompanies patients to appointments. Collaborates with primary care services.
  • Supports traditional & cultural practices: Supports patients cultural health, healing, and wellness preferences. Liaises with health care providers to support culturally safe practices. Engages with clients to explore cultural practices and promote land practices.
  • System navigation: Identifies resources and funding options. Provides follow up support to outpatients and patients discharged from acute care. Helps access community services and programs.

Perinatal and Newborn Registered Nurse

Perinatal and Newborn Registered Nurses provide medical care, screening and health promotion, advice, and supports enhanced access to healthcare services, specific to expectant mothers, parents, and their newborn babies.

Mental Wellness Clinician (Encompass Pregnancy Care Cranbrook)

The PCN Mental Wellness Clinician at Encompass Pregnancy Care Cranbrook provides proactive support, education, consultation, and referrals for pregnant or post-partum individuals experiencing mild to moderate mental health and substance use conditions including depression and anxiety.

PCN Maternity Nurse Maple Services

Working alongside the physicians at Maple Maternity Clinic in Creston, the PCN Maternity Nurse provides medical care, screening and health promotion, advice, and supports enhanced access to healthcare services, specific to expectant mothers, parents, and their newborn babies.

Respiratory Therapist

PCN Respiratory Therapists provide assessment, education, and respiratory care to pediatric and adult patients.

Service Examples:

  • Education for asthma, allergic asthma, COPD, chronic cough, fibrosis
  • Artificial airway management/support (for tracheostomy or laryngectomy patients)
  • Invasive and non-invasive ventilatory support (for bipaps and ventilated patients)
  • Sleep apnea education/support
  • Pharmacology education/support
  • Tobacco dependence counselling
  • Oxygen assessment/pre-screening
  • Respiratory device training
  • Hospital admission follow-up

Elk Valley Living Well with Persistent Pain Program

The Elk Valley Living Well with Persistent Pain Program helps individuals manage pain and return to activities they enjoy. The program combines group education classes, independent learning, and individual appointments with healthcare professionals. If you are keen to develop your self-help skills to control and relieve your pain, and to improve your function and activity, then contact your Family Physician or Nurse Practitioner about joining the program.
